New Safe Transitions of Care ToolkitMONDAY, JANUARY 15, 2018
A new resource, A Toolkit to Engage High-Risk Patients in Safe Transitions across Ambulatory Settings from the Agency for Healthcare Research and Quality (AHRQ) aims to reduce the risk of patient safety errors occurring during transitions in ambulatory settings. Patients who transition from one ambulatory care facility clinician to another are especially vulnerable to patient-safety errors, in part due to a lack of effective communication and patient engagement in shared decision-making.
